Pensioner forms are sorted into the following three categories:
  • Forms used exclusively by a GEPF Pensioner
  • Funeral Benefit claims by a GEPF Pensioner
  • Forms  potential beneficiaries must complete in support of claims following the death of a GEPF pensioner (if any benefits are still payable);

The following forms are used by the pensioner to inform the GEPF of changes to his or her personal or contact information, or to request changes such as an additional tax deduction or a change in banking details.

Where a pensioner submits new banking details, then he or she must select the correct bank form based on whether the bank account is a local South African bank, or whether the account is with a bank outside South Africa.

These forms must be submitted directly to the GEPF as per the contact details provided on the forms.

Funeral Benefit claims by a GEPF Pensioner

A funeral benefit becomes payable upon the death of the spouse or eligible child of any pensioner who retired on or after 1/12/2002, or if the pensioner retired prior to 1/12/2002, he or she must have been alive on or after 1/4/2012.

The benefit values are outlined in the instruction to the Funeral Benefit Claim Form.

The pensioner must complete the Z300 funeral benefit claim form, as well as a Z894 bank form (for a personal bank account at a South African bank, or an overseas banks or estate late bank account).
